Complaint against pharmacy or pharmacist
If you want to file a complaint against a pharmacy or pharmacist, go to the state-specific links below. You must contact the state board of pharmacy in writing in one of four ways:
- Click on the website and look for a link to “complaints” or search for “complaint.”
- Call the board of pharmacy.
- Fax a letter explaining the complaint to the board of pharmacy.
- Email a letter to the board of pharmacy.
Describe your complaint in detail, including facts, dates and names. If the complaint is about a prescription, provide the details from your prescription label.
