Report inaccurate pharmacy information

Reporting inaccurate information in our pharmacy data helps us better serve your needs. Send an email to: provider_directory_invalid@optum.com

Choose your pharmacy - We give general information about pharmacies. We do not endorse a pharmacy, doctor, or health care worker. The directory is correct at the time of update. Pharmacies may have been added or removed from our network after the last update. To confirm if a pharmacy is in the network, call the pharmacy or the phone number on your ID card.

Paper directory - Members can get a paper copy of the paper pharmacy directory at no cost by calling the phone number on the back of their ID card. Paper copies are not available to non-members.

About network pharmacies - All network pharmacies must meet National Committee for Quality Assurance credentialing standards. They must supply updated information to Optum Rx at least every three years. If you have any questions about a network pharmacy, contact the pharmacy or call the phone number on your ID card.

Demographic information - Network pharmacies must confirm some information to Optum Rx at least every three years (name, address, phone number, gender, languages spoken) for the directory. Optum Rx reviews all this information at least every three years for credentialing.

Language - Languages, other than English, that a pharmacy or staff at a pharmacy can speak. This is reported by the pharmacy to Optum Rx at least every three years for credentialing.
